Please see the above link for information on SAR Requests
These can be obtained by giving the Medical Centre 48 hours notice. Requests can be made by telephone (between 1.00pm - 3.00pm)) by letter (enclosing a stamped self-addressed envelope) or in person.
Prescriptions can also be requested on-line, please ask at reception for details.
Please remember that only those medicines which have been previously authorised by the doctor can be obtained this way. Please also note that periodically you may have to attend the surgery to enable the doctor to decide if you should still take the same medication or any alterations should be made. The medication review date will be on the form with the prescription.
If there is a change in your state of health please seek the doctor's advice before obtaining another prescription.
Extensive exemption and remission arrangements protect those likely to have difficulty in paying charges (NHS prescription and dental charges, optical and hospital travel costs). The NHS prescription charge is a flat-rate amount which successive Governments have thought it reasonable to charge for those who can afford to pay for their medicines. Prescription prepayment certificates (PPCs) offer real savings for people who need extensive medication.
These charges apply in England only. In Northern Ireland, Scotland and Wales prescriptions are free of charge.
If you will have to pay for four or more prescription items in three months, or more than 14 items in 12 months, you may find it cheaper to buy a PPC.
There is further information about prescription exemptions and fees on the NHS website
Please allow 48 hours, excluding weekends and Bank Holidays, for your request to be processed. Any problems please telephone the surgery.
